Given the distances traversed in-game there's no way Hell isn't at least country-sized. Now of course given that Hell wiped out all mankind it would be thematically appropriate for it to be basically a subterranean almost earth-sized organism (if not As Big or Bigger Than earth given that it's also magical), but it would also be really funny if it was pretty much just the size of the United States

The Ultrakill update implies Hell is contiguous with Washington state, which I think sounds about right
